Who are you, and what is your business?

Adult Foster Care of the North Shore (AFCNS) has a mission to support adults with disabilities to remain living in the community.  AFCNS was founded in 2001 by Cynthia Bjorlie, MD, and now has a staff of 29 dedicated employees. Every one of our 350 clients and their caregivers receives regular visits and a monthly payment. Payment is funded by MassHealth.  Our home base is in downtown Gloucester.

Our clients are adults with disabilities who are living with and being cared for in a private home by a responsible adult. Each client has an AFCNS dedicated team of a nurse and a care manager.  We visit the home regularly, and the caregiver receives monthly payment (up to $1700 per month). 

Caregivers are often relatives such as parents or offspring (but not spouses). Other caregivers accept non-relatives into their homes.

How can the Chamber help you?

Spread the word. Many families are struggling to keep a loved one at home and don’t know that they could be receiving professional support and payment.

Other potential caregivers may have room in their home and their heart to assume care for an adult with a disability.

Why would you want to help?

Our goal is to keep people at home and out of nursing homes, hospitals, group homes, and other institutions. We are saving you, the taxpayer, money. Care at home is not only preferable, it is cheaper.

Who is eligible to be a client?

Clients are adults (over age 16) with a disability that requires daily personal care.  Personal care means physical help with at least one of the following: walking, eating (not just cooking), dressing, toileting, or bathing. The client must have MassHealth standard insurance. 

What are the requirements for a caregiver?  

Caregivers are carefully screened and qualified by the AFCNS team.  An AFCNS-qualified home may be owned or rented. The client must have a separate bedroom.  The bathroom may be shared.
